Example of an NDIS Plan
Each NDIS plan is different based on the person and on your own requirements. Your objectives, finances, support, and personal situation all factor into your NDIS plan. Although the National Disability Insurance Agency (NDIA) will occasionally alter the NDIS plan’s appearance, the samples given here are consistent with the subjects and subtopics the NDIA uses.

About Me
This data pertains to John, a participant, and his whole life.
Zak James lives in Suburb, Victoria with his parents, Kathy and Sam. The majority of his expenses are supported by his parents. His parents assist with transportation to day activities and both work part-time. He spends his weekends with his parents and routinely attends a Day program that is scheduled from 9 am to 3 pm on weekdays. Zak needs assistance to interact with the community and needs constant monitoring to be safe. Besides his parents, Zak doesn’t have many friends. Additionally, he has just moderate social skills. His difficulties are with sensory processing and associated attitudes.
PART 02
My Goals
This part discusses my Participant’s principal aim and other key topics I’ll be focusing on.
My Main Objective: To make my home life better
In the course of this plan, I want: support with my personal care 3 days a week. NDIS to assist pay for the skill development program to increase my independence. To maintain informal assistance, the NDIS will pay for respite.
My Second Goal: Accessing various everyday activities securely
I want the NDIS to fund 3 days a week for the Day Program during this plan. In order to help myself and others with techniques to participate in everyday activities, I also require a behavioral evaluation and management plan. Funding is required for my environmental issue evaluation and intervention.
My Third Objective: To journey independently to community gatherings
I need direct help to participate in community activities once every thirty days for the course of this plan. I also want training to increase my mobility, knowledge and abilities. In order to travel independently, I request financing for a transportation allowance.
My Fourth Objective: To improve life skills
During this plan I want: help workers in training to increase independence in daily living skills such as cooking and reading.
My fifth objective: To enhance my social and communication abilities
I want a speech therapy assessment, continued treatment, and communication assistance throughout this plan.

My Supports
These are the resources that will help and direct me as I work to achieve my objectives.
informal assistance (Friends and family)
Mother and father: Daily support with all activities, including interacting, getting about, making decisions, and maintaining finances.
Service and Community groups(Mainstream and Community Support)
This is referred to as “community and mainstream” support in the NDIS. It includes community organizations, schools or other forms of education, athletic or hobby clubs, and other government services.
NDIS reasonable and necessary support budgets
By reviewing the NDIS costs to the participant page on the NDIS website (www.ndis.gov.au/participants), you may decide how you will spend the amounts in each budget that is shown below.
Wherever a support is marked as “stated,” you must purchase it in accordance with the plan’s instructions. A “stated” support on the list cannot be amended.

- Purpose of support: Three hours a week assistance to access the community as attending Day Program, nine hours a week personal care, fourteen days a year in home respite, group program 30 hours a week, two hours a week home maintenance to consolidate informal supports.

- Daily life: To enhance Zahra’s gross motor abilities. Zahra needs to improve her gripping, manipulation, and general hand strength.
- Daily life: For Zahra to increase her vocabulary and functional speech in order to be able to reply to simple commands and answer yes-or-no questions.
- Medium to Long Term Life Goal(s): in all facets of life.
- In everyday life, Zahra has to be able to adhere to detailed instructions.
- Daily life: For Zahra to continue receiving consistent assistance from her family.
Informal, Mainstream and Community Supports

Zahra used to go to child care, but she now goes less frequently since it didn’t work. Every weekend, Zahra goes to the pool with help from her family.
